The Form of Agreement has to be completed before United Medicare can undertake processing of any DBS application or release DBS Disclosure. This is a mandatory legal requirement to ensure that confidentiality is maintained to comply with the Data Protection and also comply with the all the policies including the storage, access handling and usage retention and disposal of Disclosures and Information.
